OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a slip and fall hazard:  a) Calving barn: On April 12, 2019 and a times prior thereto employees used the hay tine attachment on a Manitou MLT telescopic forklift (telehandler, model 643-120) as a temporary, elevated work surface.  The telehandler with attachment were being used to straighten up and hold a light pole that was leaning over.  An employee climbed up between 10 to 15 feet onto the attachment of the telehandler to reattach a hook and chain that had come loose from the pole.      Note) Certification and documentation of corrective action is required for this item.
