29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section. On or about April 12, 2019, employees were exposed to a struck-by hazard while using a Dewalt grinder which did not have the safety guards in place.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan. On or about April 12, 2019, at the Texas Freefall water slide, employees were exposed to fall hazards when using pan stairs that were not fitted with wood or other solid material to the top of each pan.

29 CFR 1926.1052(b)(2): Except during stairway construction, foot traffic was not prohibited on skeleton metal stairs where permanent treads and/or landings were to be installed at a later date and the stairs were not fitted with secured temporary treads and/or landings long enough to cover the entire tread or landing area. On or about April 12, 2019, employees were exposed to fall hazards when crossing skeleton metal stair landings that were not fitted with secured covers over the entire landing area.
