29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:   On or about April 16, 2019, during the installation of roofing material at a job site located at 5411 Dressler Rd NW, N. Canton, Ohio 44720; the employer did not ensure employees working on a flat roof were protected from a fall of approximately 10.3 feet by utilizing a fall protection system by providing fall protection training or fall hazard recognition.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   On or about April 16, 2019, during the installation of roofing material at a job site located at 5411 Dressler Rd NW, N. Canton, Ohio 44720; the employer did not ensure employees working on the roof of a new canopy covering the drive through lanes were protected from a fall of approximately 10.3 feet by utilizing a fall protection system.  The K ELITE LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1925.501(b)(10) Low Slope Roofs), which was contained in OSHA inspection number 1378468, citation number 01, item number 01 and was affirmed as a final order on (4/1/2019), with respect to a workplace located at 723 Horizon Dr. Grand Junction, CO 81506.
