Employees were exposed to an unprotected fall hazard of up to approximately 16 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 16, 2019, and at times prior thereto; at a residential roofing site, employees performing roofing work climbed extension ladders to access and egress the roof areas that were not extended at least 3 feet above the upper landing surface, exposing employees to fall hazards of up to 16 feet.
