On 4/15/2019, employees from Juan Ibarra Siding Company, Inc. were performing siding operations on a residential building located at 1961 Briarfield Drive, OFallon, MO.  Employees engaged in siding activities approximately 16 feet high on a pump-jack scaffold were exposed to fall hazards.  The pump-jack scaffold system did not have mud sills or base plates to support the scaffold.

29 CFR 1926.451(e)(1):  When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used.     On 4/15/2019, employees from Juan Ibarra Siding Company, Inc. were performing siding operations on a residential building located at 1961 Briarfield Drive, OFallon, MO.  Employees did not have sufficient access to the upper working levels of the scaffold system.
