The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On and about April 15, 2019, employees reroofing a steep pitch hotel had not been trained to recognize and avoid fall hazards or to understand and follow the OSHA Standards for fall protection or the manufacturer's instructions for use of personal fall arrest systems, in that: A) The employer and employees did not understand swing fall potential; B) employees did not understand anchorage strength requirements; C) employees did not understand they needed to be protected the entire time they were on the roof; D) and did not understand the necessity to limit the slack in the lifelines.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On April 15, 2019, employees were exposed to falls while reroofing a steep pitch hotel roof when employees using personal fall arrest systems were not connected to lifeline in that: A) two employees were wearing harnesses but did not have lifelines available to them on the roof; B) the ladder used to access the roof was located in an area where none of the employees could reach it without disconnecting from their lifelines.
