Located at 11382 Lockbourne Eastern Road in Ashville, Ohio: On or about April 14, 2019, the employer did not maintain an effective safety and health program to ensure fall hazards were being identified and corrected onsite while performing roofing work.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    a. Located at 11382 Lockbourne Eastern Road in Ashville, Ohio: On or about April 14, 2019, employees were removing shingles from a two story residential roof without fall protection, thereby exposing employees to a fall hazard of approximately twenty foot.
