29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):     On or about March 6, 2019 and at times prior thereto, the employer did not conduct frequent inspections to provide a workplace were any employee, worker, laborer or mechanic employed in the performance of the contract were free from recognized hazards such as but not limited to:    a)  employees performing roofing activities were not protected from falls greater than 6 feet; and  b)  employees gaining access to the roof was not provided with ladders that were extended at least 3 feet above the upper landing surface; and  c) employees were not provided hazard recognition awareness and how to avoid recognized hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about March 27, 2019, and at times prior thereto, as the controlling and correcting authority of the jobsite the employer did not ensure employees/workers engaged in roofing activities were provided fall protection, exposing them to an unprotected fall hazard greater than 6 feet to the ground below.  An employee/worker fell approximately 12 feet and broke his back.

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization of one or more employees to OSHA within twenty-four (24) hours.    On or about March 6, 2019 and at times prior thereto, Jaquez Construction, as the controlling and correcting authority did not report or ensure that the sub-contractor onsite Efrain Vasquez reported an in-patient hospitalization that occurred on March 6, 2019, at 1420 Shirley Way, Bedford, TX 76180.  The injury was not reported to OSHA until March 27, 2019.
