29 CFR 1926.300(b)(4)(ii): The point of operation of machine(s) whose operation exposed an employee to injury was not guarded:  a) Batch plant silo 4735: on or about April 15, 2019, and at times prior, two workers were exposed to caught-in hazards as they reached into a cement mixer batch chamber with their hands and handled tools to clean the mixing blade while the guard was open with the engine running.  Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:  a) Batch plant: On or about April 15 and at times prior, workers were exposed to caught in hazards while working adjacent to the rotating, pull-to-start cord spindle of a gasoline engine that was not guarded.  Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
