29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary that enabled each employee to recognize hazards related to ladders and stairways, and that trained each employee in the procedures to be followed to minimize these hazards:   a) Job site, 940 Ohio River Boulevard, Avalon, PA: On or about April 18, 2019, during construction activities, the employer did not ensure that employees used ladders safely. Deficiencies noted during the inspection included climbing up an extension ladder while holding a large HVAC vent that did not allow them to maintain an appropriate grip on the ladder and stepping sideways onto a step ladder located on top of the roof.
