29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) David Clementz, on or about April 17th, 2019, at the location of 1440 Bellefontaine Ave., Lima, OH, 45804:  The employer did not have a plan or program to identify job sit hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury or illness. Employees performing roofing activities on a commercial structure were exposed to falls of 12 feet to the ground below.

Employees performing work in this condition were exposed to falls of 12 feet to the ground below.
