29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Roof surfaces: On or about April 17, 2019, the employer did not ensure fall protection was used while installing weather proofing material to the roof surfaces which exposed employees to falls up to xx-feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Extension Ladder: On or about April 17, 2019, the employer did not ensure the ladder was set up and used with the rails at least 3-feet above the roof surface which exposed employees to a fall of xx-feet.
