29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Building Exterior-Roof: Employees were exposed to a 20 foot fall hazard while engaged in roofing activities. Employees were removing shingles without the use of personal fall arrest system or other means of fall protection in place, on or about 4/23/2019

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.  a) Building Exterior: Employees were exposed to fall hazard while using a ladder to access the roof of a commercial building, and the ladder was placed in a location that could be displaced by work activities or clients, on or about 4/23/2019
