29 CFR 1910.333(c)(3)(i)(B): When an unqualified person was working on the ground in the vicinity of overhead lines, the person approached and took a conductive object closer to unguarded, energized overhead lines than the distance given in paragraph (c)(3)(i)(A) of this section: Note: 29 CFR 1910.333(c)(3)(i)(A)(1): For voltages to ground 50kV or below-10ft. On or about April 18, 2019, and at times prior thereto, an employee was using a 47 foot water fed window washing pole to wash the windows of the building within 10 feet of the 7200 volt overhead power lines when the pole came into contact with the overhead power lines, exposing the employee to an electrical shock hazard.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about April 18, 2019, the employer did not provide employees with training on hazardous chemicals they use such as, but not limited to; RipperII concentrated detergent, Fish window cleaning foam, Dawn concentrated detergent, WOW Natural Glass Cleaner, exposing employees inhalation and fire hazards.
