29 CFR 1926.501(b)(11):  "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On 4/23/2019, employees of Alpha Stl Services, LLC were performing roofing operations at a commercial building located at 3465 State Highway K, O Fallon, MO.  Four employees were working on the roof of the building which was approximately 9.5 feet above the ground.  The employees were not using a guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards:  On 4/23/2019, employees of Alpha Stl Services, LLC were performing roofing operations at a commercial building located at 3465 State Highway K, O Fallon, MO.  The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.
