29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    Employees were exposed to struck-by, electrical shock, and amputation hazards.  The employer had not performed an annual inspection of the lockout, blockout, and tagout program for employees engaged in performing maintenance and servicing on hard wired equipment.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation guards were not designed and constructed to prevent the operator from having any part of his/ her body in the danger zone during the operating cycle:    Employee(s) engaged in operating the Tiger saw to cut parts for the Full View door assembly located at 4170 Powell Drive, Shawnee, Kansas were exposed to amputation hazards.  The Tiger saw point of operation guard did not prevent the operator's hand from inadvertently contacting a rotating blade.
