Employees will drill holes in the steel when necessary. Employees exposed to the hazard of being  caught-in the rotating drill chuck.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  On or about April 24, 2019, in building 104, the work rest on the Milwaukee bench grinder was adjusted approximately 2 inches.  Employees exposed to a struck-by hazard in the event the wheel breaks or the product being grinded is caught between the wheel and work rest and is projected back at the operator.

Employees exposed to the hazard of being struck-by the rotating grinding wheel and flying wheel parts if it would break.
