29 CFR 1910.147(a)(2)(ii): Written procedures were not developed, documented or utilized during a non-routine Servicing/Maintenance task that did not meet the exception     (a)  DIE-FOLD WRAPPER MACHINES, PEANUT BUTTER LINE - Employees removed a metal barrier guard that covered the drive chain and sprocket and made adjustments to the rotating knife of the Package Machinery Company Die-Fold Wrappers A through P, and the employees did not use lock out tag out procedures to de-energize the machines, on or about April 17, 2019.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.    ------------------------------------------------------------------------------------------------------------------------  MODIFIED VIA ISA ON JUNE 11, 2019  FROM 1910.147(a)(2)(ii) to (5)(a)(1)     OSH ACT of 1970 Section (5)(a)(1):   The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees      (a)  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious harm to employees exposed to the in running nip points of a chain and sprocket at the Die Fold Wrapping Machine on or about April 17, 2019.
