Defective rigging equipment shall be removed from service. a. On or about April 24, 2019, employees was doing hoisting operations by hoisting trusses, the employer failed to remove the defected nylon sling from service, thereby exposing the employees to a struck-by falling objects hazard, struck-by swinging object hazard, caught in or between hazards, and crushed by hazard.

Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service. a.
