29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) On the roof, on April 24, 2019, the employer did not ensure an employee who fastened singles to a roof, with the use of a pneumonic nail gun, wore eye protection, exposing the employee to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At the site, on the roof, on or about April 24, 2019, the employer did not ensure employees who were preforming roofing work were protected from falls through the use of an appropriate personal fall arrest system, thus exposing the employees to fall related hazards.

29 CFR 1926.502(d)(6)(ii): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.  a) At the site, on or about April 25, 2019, the employer did not ensure the use D-ling clips were not connected to each other, exposing employees to fall hazards.
