29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 26, 2019, and at times prior thereto; at a residential construction site, employees performing cleaning work were not provided a protective helmet while working underneath employees roofing above at higher levels. The employees were exposed to being struck by objects and/or tools falling from a height greater than (8 to 10) feet.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about April 26, 2019, and at times prior thereto; at a residential construction site, roofing employees using nailing guns were not provided with eye protection exposing the employees to eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about April 26, 2019 and at times prior thereto; at a residential roofing site, employees performing roofing activities were not provided fall protection. Employees were exposed to an unprotected fall hazard of up to approximately 10 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being access:  On or about April 26, 2019, and at times prior thereto; at a residential roofing site, employees performing roofing work climbed extension ladders to access and egress the roof areas and were not extended at least 3 feet above the upper landing surface exposing the employees to fall hazards of up to 8-10 feet.
