29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a). Main roof of the building - On or about April 25, 2019, employees worked from the roof while installing a Thermoplastic Polyolefin (TPO) system onto the roof without the use of a fall protection system, thus exposing employees to a fall hazard of approximately 15.4 feet.

Main roof of the building - On or about and times prior to April 25, 2019, the warning line system erected around the sides of roof work surface did not consist of ropes, wires, or chains, thus exposing employees to a fall hazard of approximately 15.4 feet.

Main roof of the building - On or about and times prior to April 25, 2019, employees were exposed to a fall hazard of approximately 15.4 feet when the employer did not ensure employees were trained by a competent person in the correct procedures of the following:      1. Proper installation of Guardian Temper Roof Anchors (part #: 00455);           2. Erection of warning line systems with the appropriate materials; and      3. Proper usage of Guardian Temper Roof Anchors (part #: 00455).

South side of the main roof - On or about and times prior to April 25, 2019, a stairway or ladder was not provided to access the main roof (i.e. working surface), thus exposing employees to a fall hazard of approximately 45 inches.
