29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  JOBSITE:  The employees preparing to install a lunette window on a steep pitched (10 in 12 pitch) roof were not protected from falling between 8 feet 7 inches to 28 feet 6 inches to the ground by the use of guardrails, personal fall arrest system or other equivalent means.  On or about 04/23/19, one of employees preparing to install a lunette window on a steep pitched (10 in 12 pitch) roof slipped and fell to a lower steep pitched (10 in 12 pitch) roof then fell 8 feet 7 inches to the concrete ground. The employee sustained severe lacerations to his forehead and a severe sprain to his left knee.
