29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    a) Streeter Rd Work Site: On or about May 1, 2019 and times prior, employees were performing tree trimming operations such as cutting branches from downed limbs.  Employees did not utilize head protection while cutting branches that were above head level and while moving branches overhead.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    a) Stainless area: On or about June 19, 2019 and times prior, employees used the Delta 6" bench grinder to clean up threads on bolts.  The adjustable work rest was not adequately adjusted on the right side resulting in a 1 inch opening between the abrasive wheel and the work rest.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Shop: On or about June 19, 2019 and times prior an employee operated a Delta six inch abrasive pedestal grinder clean up threads on bolts. The tongue guard on the abrasive wheel was broken off resulting in a 1 3/4 inch opening between the abrasive wheel and the top of the bench grinder.
