29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) On or about April 8, 2019 at 7655 Progress Circle, Melbourne FL, an employee suffered a work-related injury and was hospitalized.  The employer was aware of the in-patient hospitalization on April 8, 2019 at or around 9:30 A.M. The employer notified OSHA on April 15, 2019 at 4:00 P.M. of the work related hospitalization.
