29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a)  On the upper level of the clubhouse, on or about April 22, 2019 and at times prior to, the employer failed to ensure that the fall arrest system would prevent an employee from contacting the ground level in the event of a fall, resulting in a 10-foot fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Along the exterior side of the clubhouse, on or about April 22, 2019 and at times prior to, employees were exposed to fall hazards up to 10 ft., in that the employer failed to ensure that the side rails of a 20-ft portable ladder extend at least (3) feet above the upper landing surface.
