At the construction site, the four electrical receptacle box that was not approved by listing and labeling and manufacture design, with the design and method made and used by the employer, to energize hand held power equipment exposing employees to the hazard of electrical shock.

Employees were exposed to an unprotected fall of over 9 feet to the ground below.
