29 CFR 1910.178(i)(1): Concentration levels of carbon monoxide gas created by powered industrial truck operations shall not exceed the levels specified in 1910.1000. On or about April 30, 2019 the Caterpillar PIV used for material handling operations had fuel flow issues resulting in concentration levels of carbon monoxide to be produced in excess of 50 ppm. Carbon monoxide emitted by the Caterpillar model CG55KSTR PIV was measured at over 42,000 ppm by a Wisconsin Lift Truck Corp Technician. An employee employed by a company sharing the building was exposed to 440% of the 8-hour Time Weighted Average Carbon Monoxide exposure limit of 50 ppm. The employee's calculated 8-hour Carbon Monoxide Time Weighted Average was 223.8 ppm.

Employee training shall include at least: The physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area: a) On or about May 14, 2019, employees operating the LP powered PIVs had not been trained on the hazards associated with carbon monoxide present from the incomplete combustion of the LP fuel.
