29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:  a) At the jobsite located at 7020 W. 108th St. Overland Park, KS 66211: On or about March 25, 2019 - Employees engaged in commercial roofing activities, walking and working at the edge of a flat roof (canopy - the extension over the entrance) to drop the white nylon trash to the ground, were exposed to fall hazards of approximately 20 feet from the edge of a flat roof to the ground below in that fall protection was not utilized.  b) At the jobsite located at 7020 W. 108th St. Overland Park, KS 66211: On or about March 27, 2019 - Employees engaged in commercial roofing activities, walking and working at the edge of a flat roof (canopy - the extension over the entrance) were exposed to fall hazards of approximately 15 feet from the edge of a flat roof to the ground below in that fall protection was not utilized.  29 CFR 1903.19 (d)(1) requires certification and documentation that the abatement of the above violation is complete.

29 CFR 1926.1053(b)(1): 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the jobsite located at 7020 W. 108th St. Overland Park, KS 66211: Employees engaged in commercial roofing activities, using a metal extension ladder were exposed to fall hazards of approximately 15 feet from the eave of the building canopy to the ground below. Employees were exposed to fall hazards in that the ladder was not secured to the building and did not extend at least 3 feet above the eave.  29 CFR 1903.19 (d)(1) requires certification and documentation that the abatement of the above violation is complete.
