29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:    On or about April 30, 2019, at the jobsite, where employees were exposed to a fall hazard when working from a piece of plywood that was elevated using an JLG Skytrack rough terrain forklift without manufacturer approval for use.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    On or about April 30, 2019, at the jobsite, employees were exposed to a fall hazard greater than 10 feet when working on a JLG Skytrack's platform without a fall protection system.
