29 CFR 1926.350(a)(12): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks was not in accordance with Compressed Gas Association Pamphlet P-1-1965: a) On or about May 7, 2019 in the second level loft area, employees were exposed to fire and explosion hazards associated with release of acetylene gas and the employer did not ensure cylinders were protected from objects that could produce a cut or other abrasion on the surface of the metal as required in the Compressed Gas Association Pamphlet P-1-1965 Section 3.3.8.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) On or about May 7, 2019 in the new bay area employees were exposed to trip/fall hazards related to a drainage trench in that the employer did not ensure the covering was adequately sized to ensure the covering would not displace during work activities and collapse under the footfall of an employee.
