29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers.    a.  On or about May 9, 2019, at the above addressed jobsite, employees conducting masonry operations were exposed to injuries from stepping into holes measuring four (4") inches in depth by twenty-four by twenty-four (24 x 24") inches wide, thereby exposing employees to hazards associated with trips and falls.
