29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:  Worksite located at 1609 Jaclyn Drive, Grain Valley, Missouri- Employees were exposed to overhead hazards, such as, but not limited to, falling materials, debris, and equipment from the roofers working on the  roof and employees were not protected from impact by the  use of any means of head protection.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   Worksite located at 1609 Jaclyn Drive, Grain Valley, Missouri- Employees were exposed to possible eye injuries while cutting materials with radial saws as well as using pneumatic nail guns without eye protection.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    Worksite located at 1609 Jaclyn Drive, Grain Valley, Missouri- Employees were exposed to a shock or electrocution hazards in that the extension cord being used by the employees to power the saws and other had its' outer insulation breached and the  inner wires were exposed to employees.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:     Worksite located at 1609 Jaclyn Drive, Grain Valley, Missouri- Employees were exposed to a shock or electrocution hazards in that the extension cords being used by the employees to power the saws and other equipment had their grounding pins missing, thus the path  to ground was not permanent and continuous.
