29 CFR 1926.1053(b)(7): Ladders were not provided with slip-resistant feet to prevent accidental displacement on slippery surfaces.   a) On or about January 18, 2019 - 744 County Road 87, Calera, AL, employees were exposed to fall and struck-by hazards when the ladder feet were made to be slippery by adding booties on the bottom to protect the floors from damage.  No provisions were made to counter the effects of the booties by care in placing, lashing, or holding the ladders.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a) On or about January 18, 2019 - 744 County Road 87, Calera, AL, employees were exposed to fall and struck-by hazards when employees used ladders with no weight limit labels and used borrowed ladders with defects such as, but not limited to, missing weight limit labels, damaged leg, and feet that did not perform as designed due to cardboard and tape.
