29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) On or about 05/06/2019 - Entrance Hall area, 4301 Greensboro Avenue, Tuscaloosa, AL, employees installing TPO (Thermoplastic Polyolefin) on a roof with 10 skylights (3' x 8') were not protected from falling approximately 20 feet to the ground below.
