For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, or assess the limits of the exposure to the employee from respirable crystalline silica in accordance with paragraph (d) of this section:  a. Maxx Fitness Center, Easton - On May 9, 2019, Employees were exposed to the hazards of Silica exposure due to a malfunctioning vacuum system being used when grinding flooring to remove carpet glue covering a concrete floor.
