29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.   Location: Hummock Park Housing Project Nantucket  Instances a) 1st floor, vaulted ceiling room b) 2nd floor, scaffold for framing the ridge of the roof  Employees were exposed to struck-by hazards associated with scaffolding collapse, where they were not trained to recognize or avoid deficient and inadequately constructed scaffolding that had been emplaced in/on the jobsite.

Location:  a) Second floor, Unprotected edges (3)  b) Second Floor, Unprotected Windows (13) c) Basement Stairwell Opening d) Unguarded Stairway from 1st to second floor e) Unframed Foundation for Porch on the Front of the House     Employees were exposed to fall hazards up to 13 feet to the ground while working on a residential framing job where fall protection was not utilized and windows were not guarded
