Location: Hummock Park Housing Project Nantucket Instances a) 1st floor, vaulted ceiling room b) 2nd floor, scaffold for framing the ridge of the roof Employees were exposed to struck-by hazards associated with scaffolding collapse, where they were not trained to recognize or avoid deficient and inadequately constructed scaffolding that had been emplaced in/on the jobsite.
