There were three commercial fans in use for employee personnel that were not rated for industrial use. 2. On the mezzanine in the assembly area there was a 110 volt electrical receptacle knockout box for the industrial fan that was not secured to a fixed permanent structure. NOTE: Knockout boxes were designed to be secured to a fixed permanent structure. When knockout boxes are used on the ends of flexible cords, employees gripping the box to attach or disconnect a plug may dislodge the circular scored metal portion in the box wall causing it to come in contact with the hot terminal. The result may lead to an electric shock.
