Atlas post-tensioning machine without the conveyor belts being guarded.    b) On or about May 10, 2019, and at times prior thereto, employees were exposed to amputation hazards while operating the R.M.S. 411 shear machine without the guillotine blade being guarded.    c)  On or about May 10, 2019, and at times prior thereto, employees were exposed to caught-in hazards while working on and near the P.T Atlas post-tensioning machine without a guard on the rotating shaft on the north end, upper level of the conveyor belt.    d)  On or about May 10, 2019, and at times prior thereto, employees were exposed to caught-in hazards while working on and near the P.T Atlas post-tensioning machine without a guard on the rotating shaft on the south end, upper level of the conveyor belt.    e)  On or about May 10, 2019, and at times prior thereto, employees were exposed to caught-in hazards while working on and near the P.T Atlas post-tensioning machine without a guard on the rotating shaft on the north end, lower level of the conveyor belt.    f)  On or about May 10, 2019, and at times prior thereto, employees were exposed to caught-in hazards while working on and near the P.T Atlas post-tensioning machine without a guard on the rotating shaft on the south end, lower level of the conveyor belt.
