29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about May 10, 2019, and at times prior thereto, employees were exposed to amputation hazards while operating the P.T. Atlas post-tensioning machine without the conveyor belts being guarded. b) On or about May 10, 2019, and at times prior thereto, employees were exposed to amputation hazards while operating the R.M.S. 411 shear machine without the guillotine blade being guarded. c) On or about May 10, 2019, and at times prior thereto, employees were exposed to caught-in hazards while working on and near the P.T Atlas post-tensioning machine without a guard on the rotating shaft on the north end, upper level of the conveyor belt. d) On or about May 10, 2019, and at times prior thereto, employees were exposed to caught-in hazards while working on and near the P.T Atlas post-tensioning machine without a guard on the rotating shaft on the south end, upper level of the conveyor belt. e) On or about May 10, 2019, and at times prior thereto, employees were exposed to caught-in hazards while working on and near the P.T Atlas post-tensioning machine without a guard on the rotating shaft on the north end, lower level of the conveyor belt. f) On or about May 10, 2019, and at times prior thereto, employees were exposed to caught-in hazards while working on and near the P.T Atlas post-tensioning machine without a guard on the rotating shaft on the south end, lower level of the conveyor belt.

Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a) Upper conveyor belt assembly: On or about May 10, 2019, and at times prior thereto, employees were exposed to amputation hazards while operating the P.T. Atlas post-tensioning machine without the chain and sprockets being guarded. b) Lower conveyor belt assembly: On or about May 10, 2019, and at times prior thereto, employees were exposed to amputation hazards while operating the P.T. Atlas post-tensioning machine without the chain and sprockets being guarded.

Shear machine, as it was plugged into a receptacle that was not weatherproof.
