29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about 5/14/19, on a roofing jobsite, at a house located at 123 David Ave., Buffalo, NY: Ground employees who clean up debris, bring shingles up to the employees on the roof, and walk in areas where debris/equipment could fall off the roof and potentially hit them were not wearing hardhats. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 5/14/19, on a roofing jobsite, at a house located at 123 David Ave., Buffalo, NY: Employees were engaged in roofing work on the steep roof on both the left and right sides of the house. The employees were exposed to a fall of approx. 18 ft. because fall protection was not being utilized. Pitch of the roof was approx. 10 in 12. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 5/14/19, on a roofing jobsite, at a house located at 123 David Ave., Buffalo, NY: In order to access & exit the left & right side roofs, two extension ladders were set up from the ground to the high roof on both the left & right sides of the house. The lengths of the side rails of those extension ladders that were above the gutters were approx. 27 inches (right front), 25 inches (right rear), 24 inches (left front), and 23 inches (left rear). NO ABATEMENT CERTIFICATION REQUIRED
