29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about May 16, 2019, JR Remodeling, as the controlling employer, did not ensure that the employees of DR Contracting, roofing subcontractor, were protected from fall hazards of approximately 10 feet while engaged in roofing activities.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about May 16, 2019, and at times prior thereto, JR Remodeling, as the controlling employer, did not ensure that the employees of DR Contracting, roofing subcontractor, were safely using the ladder to gain access to a commercial roof. The ladder was not secured at the top and did not extend 3 feet above the landing surface.
