29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about May 15, 2019, employees were exposed to a struck by hazard while installing siding on a residential home located at 2080 Marlow, Lakewood, 44107.  Employees were working from scaffolds, above other employees, with tools and materials that could have fallen, and the employer did not ensure that the lower employees were wearing protective helmets.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.    On or about May 15, 2019, employees were exposed to a fall of approximately 14 feet while installing siding on a residential home located at 2080 Marlow, Lakewood, 44107.  The employees were working from pump jack scaffolds and the employer did not ensure that they were using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about May 15, 2019, employees were exposed to a fall of approximately 9 feet while installing siding on a residential home located at 2080 Marlow, Lakewood, 44107.  The employees were working using a ladder to access the front porch roof and the employer did not ensure that it extended at least three feet above the landing area.
