29 CFR 1910.179(j)(2)(iii): Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not performed: (a) Black Mountain Manufacturing, Inc. at 21662 W CR 52, Building 2 in Greeley, CO 80631: On or before May 9, 2019 the employer did not ensure that monthly inspections were conducted and certified for hooks that were used to lift material and equipment by the 5-ton, 1-ton, and 4K cranes in the facility. This condition exposed employees to struck-by hazards.

29 CFR 1910.179(m)(1): A thorough inspection of all ropes was not made at least once a month and a certification record which included the date of the inspection, signature of the person who conducted the inspection and an identifier for the ropes which were inspected was not kept on file where readily available to appointed personnel: (a) Black Mountain Manufacturing, Inc. at 21662 W CR 52, Building 2 in Greeley, CO 80631: On or before May 9, 2019 the employer did not ensure that monthly inspections were conducted and certified for all (wire) ropes that were used to lift and lower material and equipment by the 5-ton, 2-ton, and 4K cranes in the facility. This condition exposed employees to struck-by hazards.

Examinations were not conducted at least daily for operation of the electrical forklift and the JLG rough terrain forklift
