29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a)On 5/7/2019 a Maintenance Tech was diagnosing an issue with the tool changer of a I400 Mazak Machine in the facility and was not required to: 1) Turn off and shut down the power to the I400 Mazak. 2) Physically locate and operate energy isolating devices in such a manner as to isolate machine or equipment from the energy source. 3) Affix lockout or tag-out devices to energy isolating electrical device(s); and 4) Verify that isolating and de-energization of the machine or equipment of the machine or equipment have been accomplished.

29 CFR 1910.147(e)(2)(i): The work area shall be checked to ensure that all employees have been safely positioned or removed. a)On or about 5/7/2019 a Maintenance tech was in the Process of diagnosing an issue on a I400 Mazak CNC machine. The (MT) was in the process of trouble shooting the issues when the operator walked up to the machine, tried to press the button to open the doors, the operator instead pressed the tool engagement button, as a result the (MT) right hand was pinched in the machine.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a)On or about 5/7/2019 a Maintenance Tech was in the process of diagnosing an issue with the I400 Mazak, the (MT) removed the cover to access the tool changer area, while trouble shooting, the machine operator tried to open the doors to the CNC machine, but instead engaged the tool changer, resulting in the MT's right hand being pinched in the tool changer.
