29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about May 16, 2019, the Haas SL-40 CNC Lathe (model no. SL-40, serial no. 3089788, company no. L13) had by-passed interlocks which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips. (b) On or about May 16, 2019, the Haas SL-30 CNC Lathe (model no. SL-30T, serial no. 72951, company no. L12) had by-passed interlocks which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips. (c) On or about May 16, 2019, the Haas VF-7 Vertical Machining Center (model no. VF-7/50, serial no. 1098819, company no. L4) had by-passed interlocks which allowed employees to open the doors while the machine was in operation and exposed employees to rotating parts and flying chips.

BS-712M, serial no. 11115728) which feeds Machine L11, thereby exposing employees to crushing hazards and amputation injuries. (b) On or about May 16, 2019, employees were able to access the point of operation that was not guarded on the Baileigh Horizontal Ban Saw (model no. BS-712M, serial no. 11115727) which feeds Machine L7, thereby exposing employees to crushing hazards and amputation injuries. (c) On or about May 16, 2019, employees were able to access the point of operation that was not guarded on the Drake Horizontal Ban Saw (model no. SE712, serial no. 87B0103) which feeds Machine L6, thereby exposing employees to crushing hazards and amputation injuries.
