29 CFR 1910.272(j)(1): The employer did not develop and implement a written housekeeping program that established the frequency and the methods determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other expose:    (a)Nutraferma, Inc. at 501 South Flynn, North Sioux City, SD 57049: On or about and at times prior to May 10, 2019, the employer did not ensure employees were protected from fire and explosion hazards. The employer had not appropriately developed and implemented a written housekeeping program to reduce accumulations of grain dust. Fugitive grain dust accumulations were observed on the first three floors of the tower area of the facility between 1 inch to 6 inches in depth. This condition exposed employees to fire and explosion hazards.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either:    (a)Nutraferma, Inc. at 501 South Flynn, North Sioux City, SD 57049: On or about and at times prior to May 10 , 2019, the employer did not ensure employees were protected from chemical and physical hazards associated with employee use of isopropyl alcohol (IPA)  dispensed from inappropriately labeled spray bottles. The bottle was labeled as alcohol and did not include other health or safety information. This condition exposed employees to chemical and physical hazards.
