29 CFR 1926.1419(f): During operations requiring signals, the ability to transmit signals between the operator and signal person were not maintained. When that ability is interrupted at any time, the operator did not safely stop operations requiring signals until it is reestablished and a proper signal is given and understood. At this work place, an employee conducted block and tackle rigging operations to lift a vibro piling driver and the operator did not stop to ensure the proper signal was given to prevent hazardous caught-in conditions.
