29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system :    a- Roof Exterior: An employee engaged in roofing activities on a 7 on 12 pitched roof was exposed to fall hazards of approximately 16ft to the concrete below as a means of fall protection was not utilized. Condition noted on or about 05/20/2019.   b- Garage Exterior: Employees engaged in roofing activities on a 4 on 12 pitched garage roof were exposed to fall hazards of approximately 12ft to the concrete below as a means of fall protection was not utilized. Condition noted on or about 05/20/2019.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a)     a- Roof Exterior: An employee engaged in roofing activities on a 7 on 12 pitched roof was exposed to fall hazards of approximately 16ft to the concrete below as an extension ladder used for access whose side rails did not extend 3ft above the upper landing surface. Condition noted on or about 05/20/2019.
