29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person. On or about May 16th, 2019, employees were exposed to falls up to 17 feet to the ground while working on pump jack scaffold that was not assembled under the supervision of a competent person; where the following defeciences were identified: Instances: 1: the 20 foot long aluminum poles were out of plumb by three inches over the bottom six feet 2: the poles were not secured to the structure by rigid triangular bracing or equivalent to prevent displacement at or near the base of each pole 3: The 16 foot pump jack scaffold plank extended more than 18 inches over the supports, it was not designed and installed so that the cantilevered portion of the platform was able to support employees without tipping, and guardrails were not installed to block employee access to the cantilevered end

Location: On the Scaffold On or about May 16th, 2019, employees were exposed to falls up to 17 feet to the ground while working on pump jack scaffold where fall protection was not utilized
