Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly: (a) On or about May 20, 2019, employees were exposed to a struck-by hazard due to the use of boom attachments that were used to lift and carry stone slabs without the forklift manufacturer's prior written approval. (b) On or about May 20, 2019, data plates were not changed to reflect the capacity and operation of the forklifts with the use of the boom attachments.
